Surgery For Pelvic Organ Prolapse
When nonsurgical treatment for pelvic organ prolapse is unsuccessful in alleviating a patient’s symptoms, the choices available may sometimes be overwhelming.
Understanding the surgical options available to treat pelvic organ prolapse, the pros, cons, risks and benefits, is crucial, before making a decision regarding surgical repair.
There are several types of procedures available to treat pelvic prolapse. When selecting a particular procedure, all areas of weakness in the pelvic floor must be evaluated prior to surgery being planned.
Procedures available to treat prolapse, from lowest failure rate to highest include:
Le Fort or modified Le Fort colpocleisis, which involves inserting a barrier within the vagina to prevent tissues or organs from protruding out of the vaginal opening.
Sacrocolpopexy, which can be performed via a regular abdominal incision, a laparoscopy or robotically.
Sacrospinous Ligament Fixation, which is typically performed with a vaginal hysterectomy if the uterus is in place. It involves a vaginal approach, attaching the top of the vagina to tendons located deep within the pelvis to support the upper vagina.
Uterosacral ligament suspension is usually performed via a vaginal hysterectomy. If the uterus is in place, it involves a vaginal or a laparoscopic approach.
Anterior colporrhaphy is vaginal repair performed to replace the bladder side of the vagina into its proper position.
Posterior colporrhaphy is vaginal repair performed to replace the rectal side of the vagina into its proper position.
Each of these procedures has value for specific individual cases. In addition, each OB-GYN will have different comfort levels while performing the various procedures.
If you are contemplating prolapse surgery, it’s important to discuss your options with your OB-GYN to determine which procedure would be the best option for you.
